Recommendations by International Celiac Societies and Associations regarding diagnosis of celiac disease

Background:
The diagnosis of celiac disease (CD) is based on demonstrating villous damage and inflammation on the small intestinal biopsy. The serological (blood) tests have become an important tool in screening individuals with suspected CD. There is concern that individuals are being diagnosed with CD and treated based on serological testing alone and without a biopsy.
Objective:
To examine the recommendations regarding diagnosis of CD by international celiac societies and gastroenterological associations.
Methods:
An extensive review of all available English language web sites of international celiac societies and gastroenterological associations was conducted in November 2003. Information was specifically sought whether any group recommended diagnosing and treating CD based on serological testing alone.
Results:
No celiac society or any gastroenterological organization recommends that the diagnosis of the CD be based solely on serological testing.
Conclusion:
A small intestinal biopsy still remains the confirmatory test for the diagnosis of celiac disease.
